2012 was a productive year and we are pleased to take this opportunity to highlight the changes in policy and plans, the execution of projects and delivery of programs that occurred.
Regardless of initiative, from Complete Streets to gardens to electronic health records, the progress made this year could not have occurred without the partnerships that were created and/or strengthened by each Heart Network program. Please continue reading to learn more about our efforts over the past 12 months in Clinton, Essex, Franklin and Hamilton counties to reduce tobacco use, and increase physical activity and healthy eating – the top 3 preventable causes of chronic disease.
