Recently, the North Country Tobacco Cessation Center (NCTCC) has been collaborating with health care providers in Clinton, Essex, Franklin and Hamilton counties to incorporate evidence-based tobacco prompts into their EMR systems. NCTCC can make recommendations to modify your EMR systems to satisfy Medicaid and Medicare reimbursement requirements , Meaningful Use, Joint Commission, and Medical Home standards.
Referrals to NY State Quitline
In addition to evidence based prompts, NCTCC has helped several different providers incorporate a process which allows them to more easily refer smokers interested in quitting to the New York State Smokers Quitline. This program links patients who use tobacco to the evidence-based counseling services of the Quitline within 72 hours. By using this program, offices will be able to arrange follow up treatment for their patients easily and seamlessly while meeting the Joint Commission voluntary TOB 4 measure.
