The Get Healthy North Country Community Integrated Health Network is a coalition of medical and behavioral health providers, public health units, area offices for the aging, social care agencies, and other interested organizations that collaborate to bring health and wellness support programs to the people that need them most. The network includes nearly 30 member organizations and a growing number of program “hosts” from across nine counties in northern New York.
What We Do
Members of the Get Healthy Network offer programs that help people live healthier, more connected lives. The programs are evidence-based, meaning they’re proven to help people build confidence in their ability to manage their health. They can also help people more generally stay active and engaged in their lives. These programs are especially good for people who want to prevent or learn more about how to better manage diseases like diabetes (“sugar”), high blood pressure, anxiety, depression, arthritis, or chronic pain – just to name a few.
Value for Participants
When participants finish a program, they leave with new and/or improved life-management skills. These skills can include goal setting, decision making, strategies for dealing with common symptoms, using alternative techniques to manage symptoms, planning for the future, and improvement of interpersonal relationships. Self-management programs can also help people be better communicators, make healthier food choices, increase physical activity, improve strength and energy levels, manage symptoms, set goals, and make personal health improvement plans.
Value for Hosts
It’s not just individuals who benefit from these programs; host organizations also see tangible and intangible gains. Your organization will get access to potential new clients, garner positive media attention, and receive free health and wellness program resources and technical assistance. By participating with us, you are adding value to the scope of service and support you offer the individuals you serve. You are also adding value by making it easier for them to get easy access to these valuable services.
Get Involved
Our network is looking for organizations to serve as “Program Host Sites.” If your organization works with underserved populations, works with individuals and/or families on Medicaid, or aims to help people live fuller, healthier lives, we want to parter with you.
Program Host Sites provide meeting room space for information sessions and/or programs, share program information with the population they serve, and help people sign up for programs.
