Chronic Disease Prevention Coalition

In 2015, the Heart Network launched a new pilot program, Moving Forward Together (MFT) to Prevent Diabetes, with a three-year grant from Excellus BlueCross BlueShield. Now in its third and final year, MFT is producing results so compelling that Adirondack Health pursued and received a grant from Adirondack Health Institute to continue the program.

In addition, the Heart Network was awarded a NYS Rural Health Network Development grant to establish the North Country Chronic Disease Prevention Coalition. The Coalition brings together providers and other key stakeholders in Clinton, Essex, and Franklin counties, using the Four Domains of CD Prevention as a model to replicate MFT around the region.
