Moving Forward Together to Prevent Diabetes in Franklin County, MFT, is a new, 3 year pilot project funded by Excellus BlueCross BlueShield. MFT is testing a population based intervention used for treating tobacco use, known as the 5A’s, to see if it is effective in preventing diabetes.
The Heart Network is piloting the program first at Adirondack Health’s Tupper Lake Health Clinic. The first four months of this grant (Sept – Dec 2015) focused on planning. On December 23rd, Andrea Goff, MFT Coach began spending two days a week at the Tupper Lake Health Center. She saw her first newly diagnosed pre-diabetes participant during the first week of January. The Heart Network is very excited by the opportunity to try out a new treatment protocol to prevent diabetes.
Currently, services are only available to patients who have been referred by providers at the Tupper Lake Health Center.
