North Country Healthy Heart Network is now offering diabetes prevention coaching to pre-diabetic patients at Adirondack Health’s Lake Placid Health Center. The service has been available to patients at the Tupper Lake Health Center since last year. Expansion to the Lake Placid Health Center will increase access for Adirondack Health’s Saranac Lake/Lake Placid patients.
The service, called Moving Forward Together to Prevent Diabetes, is made possible with a grant from Excellus BlueCross BlueShield. Now in its second year, the service has already demonstrated its ability to successfully reduce participant risk factors for developing type 2 diabetes. Over 60 percent of participants have lost weight since joining the program. Six have reduced their A1C levels to a healthy range – effectively reversing their pre-diabetes diagnosis.
In addition to diabetes prevention coaching, Heart Network staff also works with Adirondack Health and North Star Behavioral Health Services to support establishment of clinical protocols. These protocols pro-actively identify patients at risk for pre-diabetes, cue providers to order diagnostic testing and connect those diagnosed with programs and services that can help.
