Our partner Adirondack Health recently received a grant from the Adirondack Health Institute (AHI) to expand and continue a program originally launched by the hospital and the Heart Network, Moving Forward Together (MFT) to Prevent Diabetes.
The AHI grant will help Adirondack Health further develop a program that will provide health coaching support for individuals wanting to reduce their risk of developing chronic disease, including diabetes. These coaches will offer prevention and lifestyle improvement education and support, with a goal of keeping healthy patients healthy.
MFT was funded by a three-year grant from Excellus BlueCross BlueShield and resulted in positive health outcomes: 36 percent of patients in the program with pre/post lab values were able to lower their blood sugar levels back into the normal range, while 73 percent lost weight.
“We’re so pleased to see this work continue at Adirondack Health,” Heart Network Executive Director Ann Morgan said. “Over 400 pre-diabetic patients received health counseling because of MFT, and we expect that number will grow with the same strong results as this program continues.”
