The North Country Chronic Disease Prevention Coalition has teamed up with Hixny to link health care providers with critical information on chronic disease identification.
Hixny is the health information exchange serving Eastern New York, from Canada to New York City and from Binghamton into Vermont. More than 1,700 locations and 6,900 users access real-time patient data through Hixny.
“Hixny will have a key role in the Chronic Disease Prevention Coalition’s work plan moving forward,” said Ann Morgan, executive director of the North Country Healthy Heart Network, which facilitates the coalition. “Their diabetes registry is a tool we hope all of our provider partners will adopt as they work to increase identification of pre-diabetes and referral of patients to programs that will help them reverse that diagnosis.”
Access to Hixny’s data also provides the Chronic Disease Prevention Coalition aggregate information, allowing organizations to view pre-diabetes prevalence rates, in real time, at both the county and zip code level.
“Access to Hixny’s data is especially important in areas that are less densely populated — like the North Country of New York — where the information available, until now, has been hyper localized,” said Tavia Rauch, Hixny’s Director of Strategic Initiatives. “The chronic disease registry is just one of the ways Hixny helps support a better understanding of the health of the entire region’s population.”
“Health systems partners will be able to track progress on preventing and reversing chronic disease in real time,” said Danyelle Jones, practice enhancement specialist at the Heart Network.
The North Country Chronic Disease Prevention Coalition brings together providers and other key stakeholders in Clinton, Essex, and Franklin (CEF) counties to identify gaps, plan, develop, and support implementation of specific chronic disease prevention programs and activities. The initial focus is prevention of diabetes.
Members of the Coalition collaborate to promote access to evidence-based prevention programs that target individuals at increased risk for developing diabetes, while at the same time supporting expansion of environmental strategies that benefit the entire population. The result will be the establishment of a system of care that emphasizes prevention — not just treatment — of chronic disease.
