2000

The North Country Healthy Heart Network, Inc. (Heart Network) is created by Walter Becker, Administrator, Carthage Area Hospital, (CAH) with a start-up grant from the Fannie E. Rippel Foundation.

2001

Mobile, one-on-one, evidence-based, tobacco cessation service begins in Franklin, Jefferson, Lewis and St. Lawrence counties. Joint CAH and Heart Network Program is made possible by a three year grant from the New York State, Department of Health, Tobacco Control Program.

2004

The North Country Tobacco Cessation Center established as a resource for healthcare providers and tobacco users in Clinton, Essex, Franklin and Hamilton counties. The Heart Network receives five year grant from the NYS Tobacco Control Program to fund this effort.

2006

The Eat Well Play Hard North Country Coalition begins efforts to prevent childhood overweight in Essex, Franklin and Hamilton counties. This program is funded by a four year grant from the NYS Department of Health, Division of Nutrition.
